School immunization requirements exist to protect students and members of their community from serious vaccine-preventable diseases by ensuring high vaccination rates.

Salem Public Schools’ Immunization of Students Policy #5703 requires compliance with current Massachusetts Department of Public Health recommended immunization schedules for school entry and attendance. 

  • All students enrolling in Salem Public Schools must provide an updated physical exam report that includes a current immunization record to gain entry to school. 
  • Current students who are not in compliance with state regulations will be excluded from school until the necessary immunizations are obtained and documentation is provided, unless there is a medical or religious exemption. 
  • Parents/guardians claiming a medical or religious exemption must contact your school nurse for instructions on submitting the required documents.
